By signing up for the HR service you agree that this initial term is for 1 year of service that will auto renew on the anniversary of the sign up date. Cancellations must be provided in writing to info@optimabenefitsgroup.com prior to the renewal date. Once you are signed up or renewed you are signed up for that year with no refunds allowed.

Payment Schedule

Deductions will occur on the 1st of the upcoming month and the 1st of each month thereafter.

Cancellation

I understand that I may revoke this authorization by contacting info@optimabenefitsgroup.com at least 10 days before the next scheduled payment.

Charges will appear on your bank statement as "DiCesare and Associates"
